Information and Precautions

We know these are challenging times for you and your loved ones. Like you, our utmost concern is for the health and well-being of each of our patients and to ensure our staff is healthy and able to continue to care for patients. With that being said, we continue to take all necessary cautionary measures and are strictly adhering to and implementing all CDC recommendations and guidelines. We continue to also work with local and state agencies as we fight Covid19. During this time we also recognize how important it is to keep the lines of communication open to give you peace of mind while we continue to meet the care needs of your loved ones while at our facility. As such, the facility will be providing updates of new developments as they take place on this page to keep you updated.

We have learned that 2 Residents have tested positive for COVID-19.  Both residents are experiencing mild symptoms and have been moved to an isolation room.  As such, we are continuing to work with Public Health and various local agencies to stop the spread of COVID-19.

Important update for Los Angeles County Skilled Nursing Facilities.

Beginning on 12/7/2022 Wednesday, ALL VISITORS ARE REQUIRED to test within 24 hours before their visit with PCR COVID testing. If testing is not done prior to arrival at the facility, the facility can offer over-the-counter (OTC) antigen tests for visitors to self-test prior to entry UNTIL SUPPLIES LAST. If visitors have their own OTC antigen tests, please bring them to the facility and administer the test with the supervision of a staff member. Any visitor with a positive test result, regardless of symptoms, should reschedule their visit until after they have completed isolation.

Please allow this email communication to serve as notification that 4 more of our residents and 3 more employees tested positive for COVID-19 within the last 72 hours.  As such, they are being closely monitored. We are also keeping in close contact with Public Health with this new development.

At present, those residents that are exhibiting symptoms are being kept in isolation. All employees that are exhibiting symptoms are being quarantined in their homes.

Like the rest of the COVID-19 positive residents, they are being quarantined from other residents. We have dedicated staff to care for quarantined residents and will monitor their health closely. As such, we are continuing to work with Public Health and various local agencies to stop the spread of COVID-19.

COUNTY OF LOS ANGELES DEPARTMENT OF PUBLIC HEALTH ORDER OF THE HEALTH OFFICER Health Officer Order for the Control of COVID-19: Prevention of COVID-19 Transmission in Skilled Nursing Facilities

ORDER OF THE HEALTH OFFICER FOR CONTROL OF COVID-19

Prevention of COVID-19 Transmission in Skilled Nursing Facilities Revised Order Issued: December 3, 2021 Effective as of 12:01am on Wednesday, December 15, 2021 This Order supersedes the April 21, 2021 Prevention of COVID-19 Transmission in Licensed Congregate Health Facilities Order, as it relates to Skilled Nursing Facilities. This Order is in effect until rescinded by the Health Officer.

Due to the introduction of the newly identified Omicron variant of the SARS-CoV-2 virus in Los Angeles County, California and other states and until more information is known about this variant’s transmissibility, its impact on vaccine effectiveness and breakthrough infections, including in individuals who have received booster doses, whether it is associated with more severe disease, and whether it is susceptible to currently available COVID-19 treatments, additional infection control precautions are indicated and necessary for Skilled Nursing Facilities because residents are at heightened risk for poor health outcomes should transmission occur within the facility. Beginning December 15, 2021 through January 31, 2022, all Skilled Nursing Facility residents, employees, and contractors, regardless of vaccination status, who may encounter residents must test for COVID-19 infection on a once weekly basis. In addition, to obtain entry into a Skilled Nursing Facility, all visitors, regardless of vaccinations status, must provide proof of either:

  • A negative PCR test taken within 72 hours prior to entry, OR
  • A negative Antigen test taken within 24 hours prior to entry.
  • Antigen test kits will be made available to Skilled Nursing Facilities on an as needed basis.

Those who show documentation of recovery from COVID-19 within the last 90 days (i.e., a positive COVID-19 viral test result on a sample taken no more than 90 days before) are exempt from weekly testing and from showing proof of a negative viral test for entry. Summary: During a State of Emergency, California law empowers the County of Los Angeles Health Officer (Health Officer) to take necessary protective measures to protect the public from a public health hazard. The spread of COVID-19 remains a substantial danger to the health of the public and to patients/residents in licensed skilled nursing facilities (SNFs) in Los Angeles County. Infection control, regular testing of residents and employees, screening testing for all visitors and social (physical) distancing measures are needed to stem or slow the spread of COVID-19 and decrease the risk of transmission to vulnerable patients/residents in high-risk health care settings. Accordingly, the Health Officer hereby requires all licensed skilled nursing facilities (“Facility”), operating within the Los Angeles County Public Health Jurisdiction, to comply with all measures in this Order. The required measures are intended to reduce the transmission of COVID-19 in these facilities and to protect vulnerable residents, as well as personnel. This Order is effective immediately and will remain in effect until rescinded, superseded, or amended in writing by the Health Officer.

Broadway Healthcare has learned that 1 employee has tested positive for COVID-19. Currently, the employee is not experiencing any symptoms but will be quarantined at home away from the facility for the next 10 days. We are closely monitoring all staff and residents for any signs and symptoms of COVID-19. Early this morning, Broadway Healthcare just learned that one of our staff members started to have symptoms related to COVID-19. A PCR test has been performed on the staff member and is currently isolating at home.

As of yesterday March 2, 2021, we do not have any COVID+ patients or employees in our facility. We are hopeful to continue to be Red Zone free and will continue to follow all of the guidelines given to us by the CDC, LACPH and DHS. We will still monitor all of our patients for signs and symptoms and screen all of our employees and vendors who come into the facility. We will be starting to do patio visits again soon. Please feel free to call the facility and ask for Amelia, our Activities Director, to schedule a patio visit with your loved ones.

We will also be slowly opening up to group activities and group dining within the regulations we are allowed. Please continue to bear with us and be mindful of the hard work and diligence of our staff to continue to do their jobs in caring and protecting your loved ones from this virus. Our hope is to get back to how things were prior to last March safely and with the guidance of those enforcing the regulations.

These new positive patients were also newly admitted from the hospital and were serving their 14 day quarantine period in our yellow zone. In total, we have 6 patients in our red zone.

The employee has since been self-isolating away from the facility and we are continuing to monitor all the residents in our facility especially those who are currently positive in the red zone.

Yesterday we learned that 1 more resident tested positive for COVID-19. This new positive patient was also newly admitted from the hospital and was serving their 14 day quarantine period in our yellow zone. In total, we have 6 patients in our red zone.

Over the past 2 weeks since our December outbreak, we were able to close the outbreak. Unfortunately, we got 2 positive results for residents that were new admissions. We also had 2 per diem dietary staff test positive, 1 who is back after self-isolating and 1 in which we got the result earlier this week. We also got word that another patient tested positive and we will continue with a Red Zone. In total, we have 2 positive patients in the Red Zone that have been/are being closely monitored. We also have 1 positive staff who is home self-isolating. Public Health is aware, and we have kept up our contact with them in regard to these positive residents and staff and are abiding by their regulations to continue to stay compliant.

Per CDC Guidelines and direction of Department of Public Health and The Public Health Department, we continue to monitor all staff and residents for sign/symptoms of Covid-19, testing is still on-going at the frequency required by the CDC and Public Health Department.

We continue to monitor all staff members before and after each shift, which includes taking and documenting each person’s temperature. We continue to adhere to infection control guidelines as outlined by Public Health.

December 24, 2020

Over the past week and a half since our 2 positive staff we have had a handful of residents and staff turn up positive. In total, we have 12 positive patients in a red zone and are being closely monitored. We also have 4 positive staff who are home self-isolating. Public Health is aware, and we have kept up our contact with them in regards to these positive residents and staff and are abiding by their regulations to continue to stay compliant.

Per CDC Guidelines and direction of Department of Public Health and The Public Health Department, we continue to monitor all staff and residents for sign/symptoms of Covid-19, testing is still on-going at the frequency required by the CDC and Public Health Department.

We continue to monitor all staff members before and after each shift, which includes taking and documenting each person’s temperature. We continue to adhere to infection control guidelines as outlined by Public Health.

Today we received results that 2 employees have tested positive for Covid-19. Those 2 employees are now self-isolating at home.

Per CDC guidelines and direction of the Department of Public Health and The Public Health Department, we continue to monitor all staff and residents for signs/symptoms of Covid-19, testing is still on-going at the frequency required by the CDC and Public Health Department.

We continue to monitor all staff members before and after each shift, which includes taking and documenting each person’s temperature. We continue to adhere to infection control guidelines as outlined by Public Health.

We have learned that 1 resident has tested positive for COVID-19. As such, they are being closely monitored. We are also keeping in close contact with Public Health with this new development.
